Test Forms

Per our mandates (HMKA-M7), and to ensure consistency in training and rank, clubs (and schools) are required to use the following forms for testing/promotion.

White Belt
- This test form is used for testing beginners (white belts) after approximately 6 months of training.

Green Belt
- This test form is used for testing green belts.

Green Belt Test Form

Blue Belt (First Test)
- This test form is used for testing blue belts for the first time after their promotion to blue.

Blue Belt (First Test) Test Form

Blue Belt
- This test form is used for testing experienced blue belts.

Blue Belt Test Form

Brown Belt
- This test form is used for testing brown belts.

Brown Belt Test Form

Interim Black Belt
- This test form is used for testing interim black belts.

Interim Black Belt Test Form

First Degree Black Belt
- This test form is used for testing first degree black belts.

First Degree Black Belt Test Form

Second Degree Black Belt
- This test form is used for testing second degree black belts.

Second Degree Black Belt Test Form

Levels above Second Degree Black belt require the direct involvement of a member of the board. These test forms are provided on request.

Below you will find the prior versions of the test forms. They are no longer current, and should not be used. They are simply supplied for reference to track changes as we make minor adjustments.
